Forms

The Office of Student Life (OSL) manages the student organization process through the use of online forms. These forms make student organization management more accessible and easier to track information. For your convenience, forms used by our student organizations are listed on this page.

Chartering Forms

    • This form is used by students to start a new student organization at ³Ô¹ÏºÚÁÏÍø.
    • This form is used by students returning to their leadership position in a previously chartered student organization.

Membership Related Forms

    • This form is used by our enrolled students to join an organization, if they are unaware of the organizations currently charted.
    • This form is sent in the month of May of the current academic year to help the group determine its leadership for the next year.

Advisor Forms

    • This form is for Advisors to complete when they are asked to advise a student organization.
    • This form asks returning advisors to continue their role as advisor to the student organization. This information is learned from the returning student organization form.
    • This form is to assign a new advisor after an advisor has left the position, or is removed from the position after processes outlined in the student organization manual.

Group Management Forms

    • This form is used by chartered student organizations to register their meeting times with OSL. Student organizations must hold 3 meetings a quarter.
    • This form is used for all recognized groups to continue to the next quarter. This form is required if the group wants funding for future activities.

Fund Related Forms

    • This form is used to request funding before any planning for activities or travel occurs.
    • This form is required for all fundraising activities. This includes asking student organization members for cash to support a purchase.

On-Campus Activity Forms

    • This form will register your form with OSL so you can begin planning.
    • This form helps with marketing your activity.
